Any 7th and 8th grade boy wanting to attend open gym and/or tryout for middle school basketball must have all required athletic forms completed in their FinalForms account.  If you have a current physical on file with the Athletic Department, it will be added to your account once it is created.  If you do not have a physical or it is expired, you must upload your new OHSAA sports physical to your account.

Info For Final Forms:  

FINAL FORMS
1.  Anyone attending any open gyms, practices, trying out, etc MUST have a fully compliant and complete Final Forms account.  This means 3 of the 4  icons should be green (1st, 2nd and 4th).  Only the 3rd icon ($) should be red (that will turn green when PTP payment is made).  See the screenshot below of my son's account.  

2.  The last icon box is a countdown clock showing how many days are left until the athlete's physical expires.  If this icon is red and shows a zero they either never uploaded a physical or the one that was on file has expired.  Everyone must have an UPLOADED OHSAA physical in their Final Forms account.  You should not be accepting any paper copies of anything.  Uploading the physical is simple.  They only need to upload (2) pages - the physical examination form and the medical eligibility form.  These are the two forms the doctor completes and signs.  The pages can be uploaded by scanning into a PDF or simply taking a picture of the two pages side by side with their phone.  They then touch the file folder icon to the far right of the athlete's name, choose upload document, then upload physical, choose from files or camera roll/photo gallery and then select their document.  If a student tries to bring you their paper form, share these directions with them and have them use their phone to upload the physical right then and there.  In the rare instance that a student does not have a phone, you can check to make sure the physical is valid, allow them to participate that day only and tell them it must be uploaded by their parent that night.  Again, please do not collect any paper forms! 
3.  We should not be giving anyone the link to the OHSAA physical online b/c they then complete all the forms on paper and want to turn that in.  They need to complete all the info electronically in Final Forms.  Once their account is complete, they go back to the main page of their Final Forms account and click on PPE MEDICAL (see screenshot below).  This opens up the OHSAA physical form and has the first 3 pages prefilled with everything they just completed electronically and the last two pages are what the doctor will complete and sign.

RBC BOYS BASKETBALL OPEN GYM:
We will have 3 open Gym dates from 2:30-3:45 in the RBC Gym, Thursday October 19th, Wednesday October 25th, and Thursday November 2.  ALL PAPERWORK MUST BE TURNED IN TO FINAL FORMS, I DO NOT COLLECT ANY PAPERWORK AND YOU WILL NOT BE ALLOWED TO PARTICIPATE IN OPEN GYM OR TRYOUTS IF YOU DO NOT HAVE ALL PAPERWORK.

ALL INTERESTED 7th GRADERS INTERSTED IN TRYING OUT FOR BASKETBALL SHOULD JOIN THE GOOGLE CLASSROOM: dahaph6

TRYOUTS WILL BE MENTALLY AND PHYSICALLY CHALLENGING! 

These are a few of the things that we are looking for during tryouts. 
- The ability to focus and perform while physically exhausted. This will be tested during tryouts. If you have asthma, bring your inhaler. If you are not physically fit, all parts of your game will suffer! 
- Mental and physical toughness. Basketball is a contact sport. The game requires you to dive on the floor to get the ball, box out aggressively and fight for rebounds, set and take on picks/screens, etc. 
- Hustle     
- Good Attitude (This is something that you CAN control) No whining!
 - Quickness to cover the entire floor - Good shooting skills (on the move as well as set shooting)
 - Good passing skills 
- Good rebounding and box-out technique 
- Good ball-handling skills and ability to create a shot!
 - Smart, aggressive defense - Are you coachable (do you follow my directions readily and accurately)? 
- Decision making, with and without the ball. 
- Mature behavior and strong work ethic in the classroom (I will be in contact with your teachers!)
- Can you perform under pressure while an entire gym of people are watching you?      

-Your past accomplishments and team membership mean nothing. You will be evaluated according to your current assets and athleticism.
