Attention parents of 2020-2021 7th grade RB Chamberlin students, starting with the 2016-2017 school year, the Ohio department of Health has mandated 1 Meningococcal vaccine (MCV4) and 1 Tdap booster shot.
Proof of administration from a health practitioner is required to be turned into the RB Chamberlin Nurse’s Office. Failure to do so by September 12, 2020 will result in exclusion from school per state and federal code. See the attached memo that was sent home to all parents in April 2020.
